DIRECTIONS FOR COMPLETING THE ONLINE MEMBERSHIP FORM:

1. Click on the 'Online Membership Form', above. Save it to your computer with a file name that you can remember.
2. Open the saved file and fill out the form. Notes: Text may move as you type. Don't worry. The main point is to complete the form as best as you can.
3. Once you finish the form, save it, then print it.
4. Mail the completed form with your check to the LWVDV office (address is on the form).
